I have reviewed the Volunteer Safety Guide and acknowledge that I have read, understood, and agree to follow the safety policies and procedures described in the guide.

I have reviewed the Release and Waiver of Liability. I understand the effect of this waiver and recognize my right to seek legal counsel before working on the job site.

I understand that these requirements extend to all members of the work party that I am bringing to the job site. All members of the work party have reviewed these guidelines and I will help assure that they adhere to the safety guidelines provided.

I release Sicora, Inc., the mobile home park, and the property owner from all liability arising from injury or incident arising out of work completed through Sicora Cares.

I grant Sicora, Inc., unrestricted permission to use any pictures, video, or narrative in which I may be included through any means whatsoever for any lawful purpose whatsoever.

I further release and discharge Sicora, Inc., from any and all claims arising out of such use or activity.

Release & Waiver of Liability for a Minor:

It is the policy of Sicora Cares and Sicora Inc. that children under the age of 14 are not allowed to work on the Sicora Cares worksites while construction is in progress. It is further the policy of Sicora Cares and Sicora Inc. that, while minors between the ages of 14 and 18 may be allowed to participate in construction work, using power tools, excavation, demolition, working on rooftops and similar activities are not permitted for anyone under the age of 18. If the Volunteer is less than 18 years of age, the Volunteer and the parents having legal custody and/or the legal guardians of the Volunteer (the “Guardians”) also hereby release and forever discharge the Released Parties from any claim whatsoever which arises or may hereafter arise on account of the decision by any representative or agent of the Released Parties.

I certify that I do not plan or intend to sell my home within the next 2 years. If I sell my home within 2 years, I will reimburse Sicora Cares for the fair market value of the materials and labor provided.

I understand that Sicora would like for me to do one of the following: to assist in the completion of the work (as you are able), donate time to work being completed on another project, donate time to a Mobile Hope program, or other idea for how to serve a neighbor, as I am able. Clients over 65 are not obligated to complete this.

I understand that Sicora reserves the right to revoke acceptance of my home from this program for any reason at any time.
I understand that the people who may work on my home are unpaid volunteers. Sicora offers no warranty as to the work that is being completed other than that which may be required by MN State Statute.

I release Sicora, Inc., and all associated, from all liability arising from negligence for any personal injury or property damage arising out of or relating to the work done on my home. I grant Sicora, Inc., unrestricted permission to use any pictures, video, or narrative in which I may be included through any means whatsoever for any lawful purpose whatsoever.

I further release and discharge Sicora, Inc., from any and all claims arising out of such use or activity.

Please note that to assess your homes suitability for our program, Sicora, Inc. reserves the right to inspect the home and property surrounding it. Your signature below gives us permission to conduct this inspection.

By signing this document, I confirm that all information in this application is correct and that I understand and agree to the stated terms and conditions.
